Hello and welcome to Sadie's Safe Harbor! We are a close-knit group of volunteers who all share the same passion for helping our unfortunate canine friends. While we understand the decision to surrender a pet is not an easy one, we appreciate that you are considering us to help you through this difficult time. The following application was designed to let us gain an understanding of your pet as well as the household he/she is coming from. It is our goal to make this as easy as possible for you while also allowing us to determine the best foster placement for your pet. Please keep that in mind as you are answering these questions and please be as thorough as you can. Once we have a chance to review your application, somebody within the group will be in contact. While submitting this form shows your intent to surrender your pet, it does not serve as a guarantee that we will be able to assist you. All of our dogs live in foster homes as they await adoption and our ability to accept new dogs into our group is dependent upon many factors. As such, we ask that you please be patient while we work through this process. Upon agreement from a SSHCR leadership representative, the surrendering owner's signature of this surrender application, and physical transfer of the dog to SSHCR's care, Sadie’s Safe Harbor Canine Rescue, Inc., its representatives and designates will be the rightful owner of your former dog and you acknowledge you are relinquishing your rights as caregiver and decision maker. Additionally you will no longer be held liable for any responsibility to your dog or for their actions. We thank you for choosing rescue to help you with a difficult choice. We promise to always provide the best care for your former dog, our responsibility to them will be a lifelong commitment. By adding your name and today's date below, you indicate your desire for us to consider taking your dog and for us to make arrangements to take in your dog. You will be required to add your signature to a printed copy upon surrender.*
